MAMA Website Music for
Schools Program Application
Thank you for your
interest in MAMA Foundation's Music for Schools Program. Under
this program we provide complimentary copies of each release
in our catalog to music libraries of institutions that are in
the forefront of jazz education today.
QUALIFICATION To
qualify, our school must be either a) an accredited college or
university which offers a degree in music or musicology and which
places significant emphasis on jazz studies; or b) a high school
or a program servicing high schools which has shown an exceptional
commitment to jazz education.
TO APPLY FOR THE PROGRAM Please
print out this blank form, have the music librarian of your school
complete and sign it, and mail or fax it to us at: MAMA Foundation,
12400 Ventura Boulevard, Suite 662, Studio City, CA 91604. Fax:
(818) 985-6544. Please allow 3 to 4 weeks for processing. Or,
